In accordance to MCO 5750.1H (Page 2-5), all officers promoted to the grade of colonel are required to submit current biographical information and a photograph to the History Division for retention in the Reference Branch Biographical files.  New biographical forms and photos should be submitted with each subsequent promotion.

The goal of this program is to maintain information on the current and future leaders of the Marine Corps.   Please fill out the above form completely and re-submit it to our office either by mail or by email (see below).  If you choose to submit your photo in a digital format, please ensure it is a high-resolution scan of 300 DPI or higher (JPEG or TIFF).

All information is protected under the 1974 Privacy Law and will be used for
official purposes only.
